This form must be submitted as an application to receive funds from Ice's Beautiful Soul.

Financial information may be requested upon receipt of an application. Such information may include: vet bills or personal financial information.
The amount of funds provided will be determined on an individual basis.
To be eligible to receive funds from Ice's Beautiful Soul, the individual(s) and/or dog(s) to receive the fund, must be a current member/participant in the agility community or have been a member/participant in the past 12-24mths.
If you are completing this form on behalf of someone else, we ask that you inform the individual you wish to receive the funds, of your application to Ice's Beautiful Soul.
Eligibility will be determined following receipt of this form.
Upon receipt of this form, we will reach out to request more information.
Exceptions to the guidelines regulating the Ice's Beautiful Soul Fund will be determined on an individual basis.
